FMEA lets a team review possible failures and existing controls to choose improvement priorities. Support the result with actual records and a shared method.

Choose a process step and failure mode

Select loading, waiting or handover, for example. Record what could fail, the product or operational effect, and what evidence would show it.

Review current controls

Note prevention steps and how a failure would be detected. Lot records, deviations, maintenance and operator observations ground the occurrence rationale.

Explain the priority

Team members assess effect, occurrence and detectability using the organization's defined scale. Avoid universal scores or preset rankings; record disagreement in the rationale.

Verify the action. Assign an owner and due date to each control. After implementation, review new records and reassess whether the failure mode or detection method changed.

Keep the analysis current. Reopen relevant rows after a new product, route, equipment or deviation. Keep the change record and team approval with the analysis.
