A broken ampoule or vial creates both a glass injury and product-integrity risk. First control the area under the institution’s safety procedure and segregate potentially affected stock from usable units.
Complete the incident record
Record product and batch or lot ID, number of broken and apparently intact units, discovery time and place, box ID, sender and receiver, and photographs. Attach the relevant temperature graph, logger ID and timestamps around the event.
Manage broken glass and spillage under institutional safety, cleaning and waste instructions. Keep other units that may have been exposed separate as well. Promptly notify the carrier and responsible pharmacy or quality team about packaging damage.
Do not decide independently whether apparently intact product can be accepted, used or discarded. Obtain written direction based on the incident evidence from the manufacturer and authorised quality lead; link the decision and follow-up action to a traceable record.
