Delivery rejection is a single outcome code; unless damage, wrong goods, paperwork, timing, temperature, packaging and the receiver's capacity are separated, all loss gets assigned to the cold chain.
Tie the first observation, evidence, container state, timeline and decision for each rejection to one batch file. Where several causes exist, mark primary and contributing separately: if the temperature record is clean and the dock was outside its appointment, timing is primary and the receiver's capacity contributing.
Rejections can seem to fall only because they moved into another category, so keep the cause definitions fixed for the whole period.
