Separate product decisions from process review after a delivery error. A team discussion cannot replace an authorised quality decision.

Start with the event record. Verify times, container or shipment ID, observation and first action. Mark unknowns; follow the authorised route for product decisions.

Questions to ask

Which tasks were in progress?

Which instruction and information were visible?

What tool or work assignment might help?

How to listen

Ask for the sequence. Compare the account with records; do not infer intent. Mark unconfirmed details separately.

Records and privacy. If documentation is needed, follow company procedure. Record the event, evidence, decisions and process change without unnecessary personal detail.

Individual or process factors. Compare the step across shifts. One event proves neither worker fault nor system failure; review training, screen, instructions and workload separately.

Choose one change

If a field is hard to find, fix the form; if wording is unclear, ask the owner to clarify. Check one change on a comparable task.

Follow-up

Assign an owner and date. Record completion and new obstacles; if the change fails, review the process.

Feedback to the team

Share the process change with the relevant team. Follow company procedure for personal details.

Close-out note

Separate findings, unknowns, authorised decision, process action and follow-up. Later review can see what changed.
