A lid problem may not have a clear cause on site. Record the observation first.
Initial record
| Observation | Add |
|---|---|
| Lid will not close | Contact or gap location |
| Castor turns with difficulty | Castor and surface |
| Gasket is displaced | Location and last task |
Decision on use
Do not guess the effect on function or product safety. Follow the authorised procedure for segregation or stopping use; record the decision-maker.
Photo and context
Photograph the overall view and part with the container ID. Explain unclear scale or location; a photo alone is not diagnosis.
When the cause is unclear
If the cause is unknown, use a site status such as 'awaiting classification'. This preserves the observation for later review.
Who reviews it
Maintenance reviews against model instructions and site process. The field worker reports part, symptom, time and ID, not root cause.
A symptom report is more useful than a category presented as proven.
Describe it in four fields. Write 'K-12, gap beside left latch; pre-dispatch check, 14:10' instead of 'lid will not close'. Never guess missing time or location.
After inspection
Link inspection to the original report. Separate finding, work performed and return-to-use decision; follow procedure for interim status.
A teaching exercise
Show a mark, loose latch and closure problem in training. Ask for observations and further checks, not automatic categories.
Compare reports
Repeated symptoms can prompt technical review. They alone prove neither cause, responsibility nor financial liability.
