When returns and waste rise, first make the rate comparable. A count alone can mislead when shipment volume or product mix has changed.
Standardise the record fields
For each event, record product and lot, store or route, date and time, returned quantity, shipped quantity and reason code. If the cause is unknown, mark it “under review” instead of guessing. Track damage, date, temperature record and order error separately.
For a period rate, use consistent units: returned units divided by shipped units, or returned kilograms divided by shipped kilograms. Do not compare periods directly if the date range, product group or scope differs. Show denominator changes and missing records in the report.
If one product, route or time window shows an increase, sample its shipment and quality records. The pattern tells you where to investigate; it does not by itself prove equipment or staff fault. After making a correction, monitor again with the same metric definition.
