A recurring exception cannot be reviewed as an isolated event. First make each instance visible in a consistent record, using dates and field evidence rather than estimates.

Record event time, product or route, shift, expected practice, what happened, reason, immediate check and decision owner. Link logger reports, photos or maintenance records where available.

At a weekly or site-defined review, group repeats under the same categories. Compare procedure, equipment, workload, waiting and training with field staff; identify process friction instead of blaming an individual.

If the procedure cannot be followed, have the authorised quality or process owner review a change. Record the approved step, owner, training date and check criterion; use the same fields to monitor recurrence.

An incident record alone does not prove a product safe or unsuitable. Make product decisions separately under the applicable specification and quality procedure.
