What information and records accompany insulin handover?

Pass on product instructions, document handover and refer excursions to the pharmacist.

At handover, provide the patient with current product leaflet storage information and the pharmacy’s approved transport instructions; do not alter them. Link lot, receipt time, package condition and any transport record. For delay, suspected heat/freezing or data excursion, do not leave use decisions to the patient or courier; seek direction from the pharmacist and manufacturer.

Handover check

Record product and lot ID, package integrity, pharmacy dispatch and patient receipt times. If a courier is used, add transport packaging and custody handovers to the form.

Provide the product leaflet and a clear contact for excursion questions; before advising a refrigerator location or temperature, verify the product instructions and pharmacy procedure.

Do not decide use or disposal after delay, damage or a temperature-record deviation. Refer the patient to the pharmacist; preserve lot and records and file manufacturer or authorised quality assessment.
