Before calculating frequency, define in writing what counts as an event. Tie the definition to the applicable product or customer requirement, record source, and reviewer; do not silently reclassify earlier periods when criteria change.
For each event, record date and time, lot, route, chain stage, measurement source, data coverage, and action taken. Explain in the method whether repeated excursions on one shipment count as separate events or one event.
State the denominator beside the result: events among all shipments or among shipments with completed monitoring? Fix the period and route scope; show unmonitored or unreadable shipments separately.
Use the same definition, denominator rule, and data coverage for before-and-after comparisons. Do not combine different routes or product procedures into one average; have quality and operations owners review trends.
