A risk map should show the path a product actually follows in your operation, not an assumed chain. Start with storage, loading, waiting, transit, handoff and receipt.

At each step, record entry and exit time, product and lot, handoff owner, measurement point, instrument and existing control. If duration was not measured, label any estimate clearly.

Link recorded deviations, waits, damage or customer reports to event date and affected product. State product loss, quality review or delay outcomes with their evidence source; do not present a possible consequence as an observed event.

Have field staff and the quality owner review the map together. Examine first the steps shown to recur and affect product requirements; assign missing-data work separately where needed.

Update the map when routes, customers, equipment or instructions change. After an improvement, collect data at the same points and compare events and product outcomes.
