How to record temperature on a goods-receipt form

Record a reading with product and lot, instrument, method, location and time.

Short answer

Record product and lot, receipt time, measurement point and method, instrument ID and calibration status, result and operator. Take acceptance criteria from the product specification, not a generic limit. If there is a deviation or missing record, segregate under procedure, notify quality, and add the decision and signature to the form.

A temperature value cannot be assessed without context. A receipt form needs enough fields to reconstruct which product and conditions were measured.

Record product, lot, receipt time, instrument ID, calibration status, method and measurement location on the product. The operator should sign.

Take criteria from the current product specification or acceptance procedure. If a result is outside criteria or the method cannot be applied, flag it and request quality review.
