Swelling, a crack or unexpected wetness is a reason to investigate; by itself it does not prove the cause, fill composition or performance condition. Remove the plate from circulation, label it do not use and identify the model instructions.
In a photo-backed record, include plate ID, lot/model, container and location, observation time, and any known handling or impact. Describe potential product contact and the affected surface separately; do not open or repair the suspect plate in the field.
Do not assume safety, cleaning or reuse without the product-specific SDS and manufacturer guidance. Refer the raw observations to the quality owner for a product decision; any return of the plate to service should be documented under manufacturer assessment and site procedure.
