Package signs may justify an investigation, but cannot alone confirm thawing, refreezing or product safety. Do not substitute visual inspection for lot history.
Turn the observation into traceable evidence. Record lot and package identity, photo, receipt time, measurement location and device details. Describe clumping, leakage, distortion or ice as observations, not as a proven temperature history.

Review logger file, sensor location, starting condition and all handoff or transfer times. If records conflict with visual signs, assess whether the sensor was representative and identify data gaps; do not infer history from one spot reading.
