A fault in passive equipment may not trigger an alarm; service-ready information makes repeat problems and use decisions visible. Start each entry with the physical equipment ID.

Describe symptom and impact

Add model, container ID, component, date, location, photo and observed behavior. State separately if product transport, closure or mobility is affected.

Refer the use decision

Refer segregation, holding pending inspection or continued use to an authorized owner under relevant procedure. Do not make an unapproved repair to clear the fault.

Send a traceable service request

Provide fault history, part code, contact person and requested inspection to the supplier. Record response, work performed, part fitted and final check on the same work order.

Review recurring faults by ID, component and use area to identify spare-part or training needs from actual records.
